Tail rotor likely caused Memorial Day, 2008, helicopter crash

The National Transportation Safety Board says in a factual report that the cause of a deadly 2008 Memorial Day weekend crash was likely caused by a tail rotor failure.

Nine-year old Zachary West of Eureka, MO., died in the accident, while four others escaped the aircraft after the chopper crashed into o the lake near Sunrise Beach.
